All referral forms and patient record transfers are handled through our secure portal to protect patient privacy and ensure that clinical information reaches the right hands promptly. For urgent and emergency referrals, please call our dedicated referral specialist directly rather than submitting through the portal.
Referral Form
Use our online referral form to submit routine referrals at any time. Please include the patient's contact information, a brief description of the clinical concern, the level of urgency, and any relevant history or prior records that would support the evaluation.
Secure Record Transfer
To transfer patient records, imaging, or other clinical documentation to Boling Vision Center in advance of a referred patient's appointment, please use our secure transfer portal or contact our referral specialist directly for assistance.
